If you received Unemployment Insurance benefits, the New Jersey Department of Labor and Workforce Development (LWD) will provide you with Income Tax Form 1099-G (Certain Government Payments). However, the form will no longer be automatically mailed.
Unemployment Insurance benefits recipients will now have two options:
- View and print a copy of your Form 1099-G by logging into your unemployment benefits online account. In order to access the form on the LWD website, you will need your online user ID, password, and Personal Identification Number (PIN). If you cannot remember your PIN or are having difficulty with it, you can contact a Reemployment Call Center. The Reemployment Call Centers are:

- If you are unable to access the self-service system to obtain the 1099-G statement, you can contact your Reemployment Call Center to request a copy. The Tax Form 1099-G will be issued in January, and will cover the previous calendar year in which benefits were paid.
