Do Not Lose Your NJ FamilyCare

 

NJ FamilyCare coverage must be renewed every 12 months. You must renew on time or you will be terminated from the program and your medical care will no longer be covered.

If you have minor children, you will be notified to renew as a family. NJ FamilyCare will send you a renewal form that is already printed with the information you have given them about everyone in your family who is covered by NJ FamilyCare and your current household income. You must look over this information carefully, correct any mistakes, sign the renewal form, and return it to NJ FamilyCare as soon as possible. Always remember to save a copy for your files and to note the date you mailed it to NJ FamilyCare.

If you need assistance or you think you have been terminated wrongly or you are having problems getting the health care you need, contact the Health Care Access Project for free help through Legal Services of New Jersey’s toll-free, statewide legal hotline (1-888-576-5529).
